CONSTIPATION

Constipation is described as having three or fewer bowel movements in a week.There might be passage of hard stools or difficulty in passing stools.Though occasional constipation is very common, some people may experience chronic constipation that may affect their day-to-day life.

What causes constipation?
The food that enters the stomach, is churned and mixed and then sent to small intestine where nutrients are absorbed. The near-liquid digested food then enters the large intestine where water from food is absorbed so that by the time it reaches the end of large intestine (rectum) soft stool is formed. The slower the food moves through the large intestine due to any cause, the more water is absorbed and harder stools are formed resulting in difficult bowel emptying.

SIGNS AND SYMPTOMS:

Two or more of the following must be present for the previous three months for a diagnosis of constipation:

  • Passing fewer than three stools a week
  • Having lumpy or hard stools
  • Straining while passing stools
  • Sensation of a blockage in rectum that prevents bowel movements
  • Feeling of incomplete emptying of stool from rectum
  • Need of using a finger to remove stool from rectum or using your hands to press on your abdomen

Causes:

  • Physical Inactivity
  • Lack of dietary fiber
  • Inadequate fluid intake
  • Medications like narcotic drugs, antidepressants, diuretics, anticonvulsants, antihistaminics, iron supplements etc..
  • Irritable Bowel Syndrome
  • Hypothyroidism
  • Diabetes mellitus
  • Pregnancy
  • Mechanical obstruction like tumors in the large intestine, anal stenosis, narrowing of intestine (stricture), external compression of intestine, rectum bulge through the back wall of the vagina (rectocele).
  • Electrolyte disturbances
  • Neurologic diseases like autonomic neuropathy, spinal cord injury, stroke etc..
  • Eating disorders

Complications:

  • Piles or hemorrhoids
  • Fissure.
  • Stool that can’t be expelled (fecal impaction)
  • Intestine protrudes from the anus (rectal prolapse)
  • Ulceration of the rectum

DIAGNOSIS:

History and physical examination will usually reveal the diagnosis of constipation. Besides your doctor may suggest the following to confirm the diagnosis and to find a cause:

  1. Blood tests to look for thyroid diseases, diabetes, electrolytes etc..
  2. Sigmoidoscopy to examine the lower parts of large intestine.
  3. Colonoscopy to look for mechanical obstructions in the entire colon.
  4. Anorectalmanometry for testing the anal sphincter muscle function.
  5. Barium enema (X ray taken after a contrast enema is given)is for detecting the disease of colon
  6. Evaluation of how well food moves through the colon (colonic transit study)

Treatment:

LIFESTYLE CHANGES:

  • Exercise regularly and Increase physical activity.
  • Regular and unhurried bowel movements.
  • Do not ignore the urge to have a bowel movement
  • High fibre diet
  • Increased fluid intake

LAXATIVES:

  • Fiber supplements add bulk to stool. Commonly used ones are ispaghula, methylcellulose etc..
  • Osmotic: magnesium sulfate, sodium phosphate
  • Stimulants:
  • Stool softeners
  • Emollients
  • Enema
  • Suppositories
  • Other drugs

BOWEL TRAINING (Bio feedback)

SURGERY:

Surgery may be an option if constipation is caused by a mechanical blockage like tumors, stricuture, tumors causing external compression, rectocele, anal fissure or stricture.

Surgery can also be reserved as the last resort when all other treatment options fail, where in a part of intestine is removed.
